Composable Commerce Cloud
This is the Elastic Path hosted cloud platform (SaaS offering), which includes Product Experience Manager (PXM), Core commerce capabilities (orders, inventory, customers, etc), and Composer.

Self-Managed Commerce (Self-Hosted)
With the Self-managed, self-hosted (on-prem) product you host the system yourself, and you have full control over the infrastructure, deployment, maintenance and upgrade processes.

Self-Managed Commerce (EP-Hosted)
Instead of hosting the Self-Managed Commerce architecture yourself in your own private cloud, you can have the system hosted by by us. In this case we are in charge of hosting and maintaining the system architecture, while you have full control of the Commerce application.
